from the USO to UST to meet and assist with on-going needs of the
UST livestock research unit at (insert location) and to provide
as needed, veterinary diagnostic services. This arrangement arises
from the already close collaboration that exists between the two

of services. Toward this end, the collaboration described below
was mutually agreed upon in earlier discussions.
II. Period of agreement
The terms of this agreement shall be the period from
July 1, 2004, through June 30, 2005.

upon 60-day written notice.

shall provide research coordination, and veterinary diagnostic services
to the UST research facility at (insert location).
Research focus will be directed toward production medicine, immunology
and swine infectious disease models. All publications, reports,
abstracts, papers, etc., which result from joint collaborative work
shall give full and proper credit to both institutions and fully
acknowledge the efforts of all parties.
B. The UST swine research facility shall be fully
accessible to the collaborator, to senior veterinary medical students
from USO College of Veterinary Medicine, and to undergraduate and
graduate students seeking experience and or training in swine medicine
and research under the direction of Dr. Innovator.
C. Dr. Innovator will, as required, provide clinical
veterinary services to the private sector using fee-based models
approved and implemented by the USO College of Veterinary Medicine.
IV. Terms
UST will annually, upon billing from USO, transfer
the equivalent of (insert percentage) of the salary and benefits
for the line that supports the collaborator.
V. Evaluation
Annually, the USO requires that individuals at the
appropriate administrative level at the UST will provide a written
evaluation of the performance of the collaborator and of the services
rendered under the terms of the agreement. This evaluation should
include reference to all activities undertaken by the collaborator
on behalf of the UST, including research direction, teaching (instruction)
where appropriate and diagnostic veterinary services rendered. This
evaluation shall be completed no later than April 15 of each year.
